Court Forms
Court forms provided on this website are free to download, and are provided as PDF documents. To view PDF documents, you will need the free Adobe Reader. Paper forms can be obtained from any clerk's office. Many paper forms are free. There is a $5 charge for some forms, such as a Summons, that require the clerk's signature and court seal. Search the court forms database by clicking the button below. If you cannot locate a form contact webmaster@courts.maine.gov.
Important note: Court forms are revised by the Judicial Branch from time to time. The most up-to-date versions of forms are linked on this page. If a court form is required by Rule, Order, or Statute and an outdated version is submitted, it will not be docketed and will be returned as incomplete. To avoid incomplete filings, use the most current version of the court form.

Subpoenas
Maine attorneys can submit a request to subpoena.request@courts.maine.gov to be sent blank fillable versions of the following forms via email:
- Subpoena for Production of Documentary Evidence or Tangible Objects by a Nonparty (CR-213)
- Subpoena for Attendance of Witness (CR-072)
- Witness Subpoena for Trial/Hearing/Deposition/Inspection (CV-035)
Self-represented parties must purchase subpoenas from any clerk's office for a $5 fee (see M.R. Civ. P. 45, M.R.U. Crim. P. 17, and M.R.U. Crim. P. 17A). Find the court nearest you.
